What questions should I ask?

Feature-level satisfaction, ease of use, a missing-feature request, any bugs encountered, and likelihood to recommend.

How many questions should it have?

5–8, focused on the feature set relevant to the release being evaluated.

When / how often should you send it?

After a meaningful usage session, and again after any major release.

What does a strong result look like?
